If a Sewerage & Water Board crew or contractor has completed work near you and the work site was left in poor condition, please fill out this Report a Work Site Issue online form to request that the site be cleaned up or restored.

  • This form is for reporting quality-of-life issues at completed SWBNO work sites, such as trash, debris, equipment, barricades, or unrestored surfaces left behind. This form is NOT for reporting imminent safety hazards or emergencies.
  • To report an active water leak, sewer leak, or other field problem, including emergencies, please use the Report a Leak or Field Problem form.
